Information
Thank you for hiring me to be your Doula. Your trust in me means
more to than I can say. I am very grateful to you for inviting me to
attend you, your partner and your birth team during the birth of your
baby. My wish to you is a satisfying and wonderful birth experience.

I ask that you fill out the forms below with as much information as
you are comfortable with. The more information that I have about
you and your history the more effective my help to achieve your
birth goals will be.

After receiving completed signed forms from you, I will bring copies
of each to our next meeting for your records. Please know that this
information is completely confidential and that no one but me has
access to it with out your written permission.  The only time I will
share any of this information is with my back-up Doula, and then
again only AFTER you have given your written permission to do so.
